Clockwork Heart is a legendary chest item that raises Sentry Fire Rate for Sentry only. It lasts for the rest of the run. The item text names how far it stacks; every copy past that point is wasted, so reroll it or wish for something else.

How to get Clockwork Heart
Chest items drop from treasure chests out in the wilds — spend gold to open one, and each chest costs more than the last. Rarer items are rarer drops, so Luck (from Fortune or the Censer) makes a legendary pull more likely. On every floor’s plaza the Wishing Well grants the exact item you name once per floor: the surest way to complete a build around Clockwork Heart.

Without Sentry in your build this item does nothing, so take the skill first and wish for the item once the skill is upgraded.
